As a Seton Health Plan member, you can pick your own doctor or clinic. The doctor or clinic you pick will be the Primary Care Provider for your child. You can pick a Primary Care Provider that you have used before. Or one that is close to your home. Or one that someone you know has used. If you wish, Seton Health Plan will help you pick a Primary Care Provider.

You can also call the 24-Hour Nurse Help Line for CHIP 1-888-497-8989 and for STAR 1-855-782-4214. Someone answers that number 24 hours per day. They speak English. They speak Spanish. We can also arrange for interpreters for other languages. We can also get help for the hearing impaired.
